Can Adenomyosis Cause Thickening Endometrium?

Adenomyosis may cause the endometrium to thicken. Adenomyosis is a common gynecological disease. It is a diffuse or localized lesion caused by the endometrial glands and interstitial entering the uterine muscle layer.

Adenomyosis will cause abnormal detachment of the endometrium, and the hormone secretion in the patient's body is abnormal at this time, which may secrete estrogen and promote the growth of the endometrium. And there will be endometrial hyperplasia, which will thicken the endometrium. 


In addition, when the endometrium thickens, it may cause endometrial inflammation, and the resulting inflammation will also lead to endometrial hyperplasia. And patients will have symptoms of dysmenorrhea, heavy menstruation, and prolonged menstruation.


Endometrial thickening is caused by excessive estrogen stimulation and a lack of progesterone in the body. Therefore, it cannot be completely shed. It can be simple hyperplasia, compound hyperplasia, endometrial polyp atypical hyperplasia, etc. 


To sum up, adenomyosis may lead to the thickening of the endometrium, but not all people's endometrial thickening is caused by adenomyosis, and there may be other reasons.


Who is prone to endometrial thickening?


People with prolonged menstrual cycles during puberty, childbearing age, menopause, and some postmenopausal women. These people have a common characteristic of the lack of progesterone in the body.


Estrogen promotes the growth of the endometrium, and progesterone is resistant to this effect of estrogen. If no progesterone exists, the endometrium will first have typical hyperplasia and then excessive hyperplasia.


Excessive hyperplasia includes simple hyperplasia, complex hyperplasia, and atypical hyperplasia.


These hyperplasias are gradually aggravated by the degree of lesions, and atypical hyperplasia belongs to precancerous lesions. The excessive hyperplasia of these endometriums is first manifested in the thickening of the endometrium. But the most fundamental thing is the change in pathology, not the change in thickness.


During puberty and menopause, because ovarian function is unstable or tends to decline, it cannot ovulate normally and periodically. But the ovary can continue to secrete estrogen, so there is no progesterone to fight estrogen for a long time.


Patients with polycystic ovary syndrome are prone to excessive hyperplasia and malignant endometrium changes during childbearing age. It is also due to polycystic ovary syndrome. There has been no ovulation for a long time, but there is follicular development. Although it cannot mature, the estrogen given by multiple small follicles is enough to stimulate the growth of the endometrium.


How to prevent the thickening and malignant transformation of the endometrium?


1. Make menstruation regular


Patients with polycystic ovary syndrome in the childbearing age and women with menstrual disorders during puberty and menopause can use artificial methods to make the menstrual cycle regular and make menstruation come on time.


The simple method is to simulate the normal menstrual cycle hormone levels to give estrogen and progesterone. Or give progesterone. The endometrium falls off on time so that there is no chance of excessive growth.


2. Supplement progesterone


Progesterone can resist estrogen's effect in stimulating the endometrium's growth. This is also why we must use progesterone during the artificial cycle or postmenopausal hormone replacement therapy. Progesterone can protect the endometrium, and taking progesterone can also reverse the proliferogenic endometrium.
