Summary: Classic Traditional Chinese Medicine Formulas for Epididymitis

Epididymitis is a common condition in the male reproductive system that often becomes chronic, with the disease course lasting more than three months. The main symptoms include persistent dull pain and tenderness in the scrotum, sometimes accompanied by urinary discomfort such as frequent urination and urgency. Beyond physical discomfort, chronic epididymitis can impair quality of life, cause psychological issues like anxiety or depression, and even affect sperm quality.

In Western medicine, antibiotics remain the first-line therapy for acute bacterial infection. However, long-term use in chronic cases may lead to drug resistance and fails to address the underlying systemic imbalance. Traditional Chinese Medicine (TCM), based on syndrome differentiation and treatment, uses carefully matched herbal formulas that not only alleviate local symptoms but also regulate the whole body, reduce recurrence risk, and improve overall health. This makes TCM an important therapeutic option for chronic epididymitis.

Classic TCM Formulas for Epididymitis

TCM Syndrome Differentiation and Core Formulas

TCM attributes epididymitis to “damp-heat obstruction and organ imbalance.” Clinically, the most common syndromes include:

  • Damp-heat accumulation in the lower burner

  • Qi and blood stasis

  • Liver and kidney deficiency

Each syndrome has distinct pathogenesis, symptoms, and treatment formulas, which must be accurately matched for effectiveness.

(1) Damp-Heat Accumulation Type – common in acute and recurrent episodes

Pathogenesis: External damp-heat pathogens (e.g., unclean sexual contact, humid environment) or improper diet (spicy food, alcohol) generate damp-heat, which descends through the liver channel to the lower burner, obstructing epididymal vessels and impairing qi and blood flow.

Symptoms: Red, swollen, painful scrotum with obvious tenderness, accompanied by yellow urine, urinary frequency/urgency, sometimes turbid urine or hematuria. Tongue: red with yellow greasy coating; Pulse: slippery and rapid.

Representative Formulas:

1. Long Dan Xie Gan Tang (Gentiana Drain the Liver Decoction)

  • Key effect: Clears damp-heat from the liver and gallbladder, directly targeting the focus of infection.

  • Application: Suitable for acute or recurrent epididymitis with marked scrotal swelling, pain, and urinary discomfort. Clinical reports suggest improvement within 1–2 weeks, with consolidation therapy required for 2–3 weeks.

2. Ba Zheng San (Eight-Herb Powder for Rectification)

  • Key effect: Clears heat and promotes urination, relieving urinary tract symptoms.

  • Application: Best for patients with prominent urinary frequency, urgency, and painful urination. Often used alternately with Long Dan Xie Gan Tang for combined management of scrotal inflammation and urinary issues.

(2) Qi and Blood Stasis Type – common in chronic epididymitis

Pathogenesis: Incomplete treatment of the acute stage leaves damp-heat obstructing vessels, or prolonged sitting/trauma impedes local circulation, leading to stasis.

Symptoms: Fixed dull scrotal pain, palpable nodules (<1cm), groin pulling pain worsened by exertion or sitting. Tongue: dark with purple spots; Pulse: choppy.

Representative Formula:

Ju He Wan (Tangerine Seed Pill)

  • Key effect: Moves qi, dispels stasis, softens nodules, and alleviates pain.

  • Application: Suitable for chronic epididymitis with palpable induration and localized pain. A 4–6 week course can soften nodules and relieve discomfort, often combined with prostate massage to improve circulation.

(3) Liver and Kidney Deficiency Type – prolonged, lingering cases

Pathogenesis: Chronic disease depletes liver and kidney yin, weakening tissue repair and leading to persistent inflammation.

Symptoms: Lingering scrotal pain (worse at night), soft but persistent nodules, dry mouth, night sweats, lumbar soreness, reduced sexual function. Tongue: red with scant coating; Pulse: thin and rapid.

Representative Formula:

Zhi Bai Di Huang Tang (Anemarrhena, Phellodendron, and Rehmannia Decoction)

  • Key effect: Nourishes yin, clears deficiency heat, supports repair.

  • Application: Suitable for prolonged epididymitis with systemic deficiency signs. Typically prescribed for 8–12 weeks, improving both local symptoms and overall vitality.

(4) Multi-targeted Herbal Formula: Diuretic and Anti-inflammatory Pill

Besides classical formulas, the Diuretic and Anti-inflammatory Pill, a patented TCM preparation, is widely used due to its broad-spectrum action: clearing heat, detoxifying, promoting blood circulation, relieving pain, and improving urination.

Advantages:

  • Contains Plantago seed, Dianthus, and other herbs for urinary symptoms.

  • Includes Angelica, Carthamus, and Paeonia rubra for blood circulation and pain relief.

  • Integrates Houttuynia and Honeysuckle for anti-inflammatory and antibacterial effects.

  • Fortified with tonics like Atractylodes, Yam, Eucommia, and Goji berry for spleen, liver, and kidney support.

Clinical Use: Especially suitable for chronic, recurrent, or antibiotic-resistant cases, and for patients with concurrent prostatitis or cystitis. A 3-month course is often recommended for stable results without drug resistance.

Clinical Considerations for TCM in Epididymitis

1. Accurate Syndrome Differentiation – Misapplication of formulas can worsen symptoms. Always consult a qualified TCM physician.

2. Adequate Treatment Duration –

  • Acute damp-heat type: 2–3 weeks

  • Qi-blood stasis type: 4–6 weeks

  • Liver-kidney deficiency type: 8–12 weeks

  • Diuretic and Anti-inflammatory Pill: ~3 months for complex cases

3. Integration with Western Medicine –

  • Acute cases: antibiotics + Long Dan Xie Gan Tang

  • Chronic with BPH: α1-blockers + Zhi Bai Di Huang Tang or Diuretic and Anti-inflammatory Pill

  • Physical therapy: warm sitz baths, acupuncture, and massage enhance efficacy

4. Lifestyle Adjustments – Avoid spicy food, alcohol, prolonged sitting, or cycling; practice moderate exercise and pelvic floor (Kegel) exercises; maintain good sleep and manage stress.

5. Prevention of Complications – Regular follow-up with ultrasound and semen/prostate fluid analysis; maintain sexual hygiene; avoid both abstinence and excessive intercourse.

Conclusion

The strength of TCM in epididymitis lies in syndrome differentiation and holistic regulation. By selecting formulas tailored to each syndrome, TCM not only alleviates pain, swelling, and urinary issues, but also restores systemic balance, reduces recurrence, and improves reproductive health.

Unlike long-term antibiotics, herbal therapies avoid resistance and have milder side effects, making them more suitable for chronic management.

With proper diagnosis, sufficient treatment courses, and combined lifestyle adjustments, most patients can achieve significant improvement within 3–6 months, regaining quality of life and reproductive function.
