Herbal Remedies for Epididymitis: Relieving Scrotal Heaviness and Swelling

The feeling of heaviness in the scrotum is one of the most common and distressing symptoms experienced by patients with epididymitis. In Traditional Chinese Medicine (TCM), this condition often arises from damp-heat stagnation, qi and blood stasis, or the invasion of toxic heat, all of which impair local circulation, leading to congestion and edema of the epididymis.

Herbal Remedies for Epididymitis

The core principle of TCM treatment is pattern differentiation and targeted prescriptions—using methods such as clearing heat and dampness, promoting blood circulation, moving qi, and relieving pain to improve local microcirculation, reduce congestion, and thereby ease the sense of heaviness.

I. Core TCM Patterns of Epididymal Heaviness: Identify the Cause Before Prescribing

The type and intensity of heaviness are closely tied to different TCM syndromes. Each pattern has distinct accompanying symptoms and tongue–pulse signs, which serve as the foundation for selecting prescriptions:

Liver and Gallbladder Damp-Heat Pattern

Heaviness with burning sensation, worsens after prolonged sitting; accompanied by scrotal swelling, bitter taste in mouth, dry throat, dark yellow urine, irritability; tongue is red with yellow greasy coating, pulse is slippery and rapid.

Damp-Heat Retention Pattern

Heaviness with perineal discomfort, aggravated during urination; accompanied by painful, burning urination and sticky bowel movements; tongue is red with yellow greasy coating, pulse is soft and rapid.

Qi Stagnation and Blood Stasis Pattern

Heaviness manifests as stabbing or fixed pain, nodules may be palpable; associated with scrotal pain (worse at night), hard epididymal nodules, long disease course (>3 months); tongue is dark-purple or with stasis spots, pulse is choppy.

Excessive Heat Toxin Pattern

Heaviness with severe swelling and pain, local burning on touch; accompanied by fever (>38.5°C), scrotal redness and tenderness; tongue is crimson with dry yellow coating, pulse is surging and rapid.

II. Key Herbal Prescriptions for Epididymal Heaviness

(1) Comprehensive Regulation: Diuretic and Anti-inflammatory Pill (Multi-pattern Adaptability)

For chronic epididymitis (>3 months), recurrent heaviness, or mixed syndromes (such as damp-heat with blood stasis), a single classical formula may be insufficient. The Diuretic and Anti-inflammatory Pill (patented TCM formula) is widely used because it simultaneously addresses heat-clearing, damp-eliminating, blood-activating, qi-regulating, and pain-relieving needs.

Main Composition and Logic

Contains over 50 herbs, including Plantago seed, Talc (decocted in bag), Dianthus, Polygonum, Safflower, Peach kernel, Angelica, Red Peony, Houttuynia, Vaccaria seed, etc.

Heat-clearing & damp-draining group: Plantago seed, Dianthus, Talc → reduce scrotal congestion and edema.

Blood-activating group: Angelica, Red Peony, Safflower, Peach kernel → resolve stasis, relieve stabbing heaviness.

Qi-regulating & pain-relieving group: Vaccaria seed, costus root → restore qi flow, ease distension.

Detoxifying group: Houttuynia → inhibit residual inflammation, especially with mild urethritis.

Clinical Effects & Indications

Clears heat and dampness, activates blood, moves qi, relieves pain, promotes urination.

Especially suitable for chronic or recurrent epididymal heaviness, mixed syndromes, or patients intolerant to long-term antibiotics.

Clinical data show: after 1 month of continuous use, over 80% of patients experienced >50% reduction in heaviness episodes.

(2) Liver and Gallbladder Damp-Heat Type – Formula: Longdan Xiegan Tang

  • Core herbs: Gentiana, Gardenia, Scutellaria, Bupleurum, Rehmannia, Angelica, Alisma, Plantago seed, Akebia, Licorice.

  • Function: clears liver–gallbladder damp-heat, relieves distension.

  • Indicated for epididymal heaviness with burning, bitter taste, dark urine, irritability.

(3) Damp-Heat Retention Type – Formula: Bazheng San

  • Core herbs: Dianthus, Polygonum, Talc, Gardenia, Rhubarb, Plantago seed, Akebia, Licorice.

  • Function: clears heat, drains dampness, promotes urination.

  • Indicated for heaviness worsened during urination, burning urethral pain, sticky stools.

(4) Qi Stagnation and Blood Stasis Type – Formula: Taohong Siwu Tang

  • Core herbs: Angelica, Chuanxiong, White Peony, Rehmannia, Peach kernel, Safflower.

  • Function: invigorates blood, disperses nodules, relieves pain.

  • Indicated for chronic epididymitis with fixed stabbing heaviness and palpable nodules.

(5) Excess Heat Toxin Type – Formula: Wuwei Xiaodu Yin

  • Core herbs: Honeysuckle, Chrysanthemum, Dandelion, Viola, Begonia.

  • Function: clears heat toxin, reduces swelling and pain.

  • Indicated for acute epididymitis with fever, scrotal redness, severe pain.

  • Should be combined with antibiotics during acute infection.

III. Supportive TCM Care to Prevent Recurrence

Local Care:

  • Scrotal support to reduce sagging congestion.

  • Warm compress (40°C, 20 min) during chronic phase to improve circulation.

Lifestyle Adjustments:

  • Avoid prolonged sitting and cycling.

  • Diet: no spicy, greasy, or alcohol; eat foods that drain dampness (e.g., winter melon, coix seed).

  • Regular sleep to strengthen immunity.

Monitoring:

  • Observe symptom changes every 2 weeks; if heaviness persists or nodules enlarge, seek medical reassessment.

  • Chronic cases should undergo scrotal ultrasound every 3–6 months to check for fibrosis.

Conclusion

The TCM approach to epididymal heaviness lies in stage-based, pattern-targeted treatment:

  • Acute stages (damp-heat, heat toxin): focus on clearing heat, draining dampness, detoxifying, often combined with antibiotics.

  • Chronic or mixed patterns (qi stagnation, blood stasis): comprehensive formulas such as Diuretic and Anti-inflammatory Pill are preferred, supported by blood-activating prescriptions and local care.

Importantly, any signs of abscess formation, persistent high fever, or semen abnormalities require timely Western medical intervention. Herbal prescriptions should always be used under the guidance of a licensed TCM practitioner to ensure safety and effectiveness.
