Effective Traditional Chinese Medicine Approachs for Damp-Heat Type Chronic Prostatitis

Chronic prostatitis is a fairly common condition among men, and damp-heat type chronic prostatitis has a particularly high incidence. Men with this condition often experience frequent urination, urgency, painful urination, and a heavy or uncomfortable sensation in the perineal area, which can significantly affect quality of life. When it comes to treatment, Traditional Chinese Medicine (TCM) has its own well-established system and extensive clinical experience. Through internal herbal therapy, external treatments, dietary adjustment, and lifestyle and emotional regulation, TCM can help relieve symptoms and gradually restore health.

How Does Traditional Chinese Medicine View Damp-Heat Type Chronic Prostatitis?

In TCM theory, chronic prostatitis falls under categories such as “turbid essence,” “white turbidity,” and “exertion-related strangury.” Damp-heat type chronic prostatitis is fundamentally caused by the accumulation of damp-heat pathogenic factors in the lower part of the body. This disrupts bladder function and irritates the reproductive system, leading to a series of symptoms.

Where does damp-heat come from? It may be related to environmental factors, dietary habits, and emotional disturbances. TCM emphasizes holistic regulation, believing that proper coordination of organ function, qi and blood circulation, and yin–yang balance is essential for recovery. Therefore, treatment focuses on syndrome differentiation and overall regulation—adjusting the internal environment, restoring normal organ function, and ultimately achieving therapeutic results.

Causes and Treatment Principles of Damp-Heat Type Chronic Prostatitis

Cause Analysis

The causes of damp-heat type chronic prostatitis are relatively complex and can be divided into external factors and internal bodily factors.

External factors:

Long-term exposure to damp environments, getting soaked by rain, or wading in water can allow damp-heat pathogens to invade the body and accumulate in the lower burner. Dietary habits also play a major role. Frequent consumption of spicy, greasy, and sweet foods—such as chili peppers, fried foods, and desserts—can damage the spleen and stomach. In TCM, the spleen and stomach are considered the foundation of postnatal health and are responsible for digestion and nutrient absorption. When their function is impaired, water metabolism becomes abnormal, allowing damp-heat to accumulate in the prostate and trigger disease.

Internal factors:

Emotional disturbances are also crucial. Long-term stress, anxiety, or depression can cause liver qi stagnation, impairing the liver’s ability to regulate qi flow. As qi circulation becomes obstructed, water metabolism is disrupted, making it easier for damp-heat to develop. In addition, excessive fatigue and overly frequent sexual activity can damage kidney qi. When the kidney’s transformative function weakens, fluids accumulate in the body and gradually transform into damp-heat, becoming another contributing factor.

Treatment Principles

Based on these causes, TCM follows the principle of “clearing heat, draining dampness, relieving strangury, and resolving blood stasis.”

Clearing heat: eliminating excess damp-heat from the body to reduce inflammatory reactions

Draining dampness: promoting fluid metabolism so damp pathogens can be expelled rather than retained

Relieving strangury: easing urinary symptoms such as frequency, urgency, and pain

Resolving blood stasis: improving local blood circulation in the prostate to relieve pain and a heavy sensation

During treatment, adjustments are made according to the patient’s specific symptoms and constitution, with appropriate strengthening of the spleen and kidneys to enhance resistance and self-repair, achieving both symptomatic relief and root-level regulation.

TCM Methods for Managing Damp-Heat Type Chronic Prostatitis

1. Internal Herbal Therapy

Internal herbal medicine is the primary TCM treatment for damp-heat type chronic prostatitis. Practitioners prescribe individualized formulas based on symptoms, tongue appearance, and pulse diagnosis.

Common formulas include:

Diuretic and Anti-inflammatory Pill:

This is a traditional Chinese herbal formula specifically designed for urinary and reproductive system conditions in both men and women. It contains over 50 herbs, including Plantago seed, Dianthus, Polygonum aviculare, talc, Angelica sinensis, red peony root, and safflower. The formula clears heat and toxins, promotes urination, relieves strangury, and activates blood circulation. It is particularly suitable for damp-heat type chronic prostatitis. It can effectively relieve urinary frequency, urgency, and pain, while improving local blood circulation and reducing discomfort. Clinical use has shown favorable results, with many patients reporting good outcomes.

Longdan Xiegan Decoction:

This is a classic TCM formula used to clear liver and gallbladder heat and eliminate damp-heat. Gentiana root clears excess fire in the liver and gallbladder and removes damp-heat from the lower burner. Scutellaria and Gardenia clear heat, while Alisma, Akebia, and Plantago promote urination and guide damp-heat downward. This formula is suitable for patients with symptoms such as bitter taste in the mouth, flank pain, and genital itching.

Bazheng Powder:

Composed of Plantago seed, Dianthus, Polygonum aviculare, talc, Gardenia seed, licorice, Akebia, and rhubarb, this formula clears heat, drains fire, and promotes urination. It is particularly effective for urinary frequency, urgency, and pain, while also facilitating urine output.

2. External Therapies

Acupuncture:

Acupuncture regulates qi and blood circulation by stimulating specific acupoints. For damp-heat type chronic prostatitis, commonly used points include Guanyuan (CV4), Qihai (CV6), Zhongji (CV3), Sanyinjiao (SP6), and Yinlingquan (SP9). These points help tonify the lower burner, regulate bladder function, harmonize the liver, spleen, and kidneys, and promote dampness elimination. Acupuncture improves local circulation and helps reduce inflammation and pain.

Massage (Tui Na):

Massage techniques such as pressing and kneading applied to the perineum, lower abdomen, and lumbosacral area can promote qi and blood flow and alleviate symptoms. For example, massaging the perineum can reduce prostate congestion, while stimulating points such as Guanyuan, Qihai, and Shenshu helps regulate organ function and enhance resistance. Massage is simple and can be performed by patients at home.

Herbal Fumigation and Sitz Baths:

Herbal decoctions are used for steaming and washing the affected area. Herbs such as Phellodendron bark, Sophora root, Cnidium fruit, Salvia, and red peony root—known for clearing heat, draining dampness, and activating blood—are commonly used. Fumigating and washing the perineal area promotes local circulation and relieves inflammation, pain, and itching.

3. The Role of Dietary Therapy

Dietary regulation plays a supportive role in managing damp-heat type chronic prostatitis. A light diet consisting of easily digestible, nutritious foods is recommended. Spicy, greasy, and irritating foods—such as chili peppers, fried foods, and alcohol—should be avoided, as they can aggravate damp-heat symptoms. Foods that clear heat and drain dampness, such as winter melon, bitter melon, cucumber, coix seed, and mung beans, are beneficial. Kidney-tonifying foods like goji berries, Chinese yam, and walnuts may also be included in moderation to strengthen resistance.

4. Lifestyle and Emotional Regulation

Regular daily routines and a healthy mindset are also crucial for recovery. Staying up late should be avoided, and sufficient sleep is essential. Excessive fatigue should be minimized. Moderate exercise—such as walking, jogging, or practicing Tai Chi—helps promote circulation and improve physical strength. Emotional balance is equally important. Negative emotions can disrupt endocrine and immune function, worsening the condition. Relaxation methods such as listening to music, traveling, or talking with friends can help relieve stress.

Although damp-heat type chronic prostatitis can be distressing, TCM offers a variety of effective solutions. Through a combination of internal herbal therapy, external treatments, dietary regulation, lifestyle adjustments, and emotional management, symptoms can be relieved while the underlying constitution is improved, reducing the likelihood of recurrence.
