Top Traditional Chinese Medicine Formulas for Cystitis: Clear Heat, Relieve Urinary Pain

Cystitis is a common urinary system disorder, typically presenting with symptoms such as urgency, frequent urination, painful urination, dark or short urine, and dribbling discomfort. In Traditional Chinese Medicine (TCM), cystitis is often attributed to damp-heat accumulation in the lower burner (lower jiao). 


Using herbs that clear heat and promote diuresis is a key therapeutic approach. Here, we summarize five TCM formulas with heat-clearing and damp-draining properties that can specifically improve cystitis symptoms.


TCM Formulas for Cystitis


1. Diuretic and Anti-inflammatory Pill

Composition:

Diuretic and Anti-inflammatory Pill contains over 50 herbs, including Plantago seed (Che Qian Zi), Talc (Hua Shi), Dianthus (Qu Mai), Bi Xie, Angelica (Dang Gui), Red Peony (Chi Shao), Safflower (Hong Hua), Peach kernel (Tao Ren), Houttuynia (Yu Xing Cao), Vaccaria (Wang Bu Liu Xing), Tian Ding, Di Ding, Honeysuckle (Jin Yin Hua), among others. (This is a patented formula; specific adjustments should be made by a licensed TCM practitioner based on the patient's constitution and condition.)


Herbal Logic:

Heat-clearing and damp-draining: Plantago seed, Talc, Dianthus, and Bi Xie directly clear damp-heat from the lower burner, alleviating urinary frequency, urgency, and pain. Houttuynia, Honeysuckle, Tian Ding, and Di Ding enhance detoxification, inhibit bacterial proliferation, and address the root cause of chronic infections.


Activating blood and resolving stasis: Dang Gui, Chi Shao, Hong Hua, and Tao Ren improve local blood circulation, relieve lower abdominal discomfort, and prevent stasis caused by prolonged damp-heat.


Repair and protection: Vaccaria promotes blood circulation, diuresis, and anti-fibrosis effects, preventing trigone hyperplasia and follicle formation in chronic cystitis, thus protecting bladder mucosa.


Supporting overall health: A combination of Atractylodes (Bai Zhu), Chinese Yam (Shan Yao), Astragalus (Huang Qi), and Goji (Gou Qi Zi) supports spleen and kidney function during long-term therapy.


Effects and Indications:

Clears heat, detoxifies, activates blood, relieves pain, promotes urination, and provides anti-hyperplasia, anti-calcification, and anti-fibrosis effects. Suitable for:

  • Recurrent chronic cystitis (≥3 episodes/year), especially in patients resistant or intolerant to antibiotics.

  • Chronic cystitis with trigone hyperplasia, follicle formation, or concurrent prostatitis/urethritis.

  • Can be adjusted based on syndrome differentiation (e.g., add Huang Bai for damp-heat, Sheng Di for yin deficiency, Dang Shen for spleen deficiency).


Clinical Advantages:

Most patients experience significant relief of urinary symptoms after 1–2 months of continuous use. A 3-month course markedly reduces recurrence rates. As a pure herbal formulation, it avoids antibiotic resistance and has minimal side effects; mild diarrhea in some patients can be alleviated by taking the pill after meals.


2. San Jin Granules (Powdered Formula)

Composition: Jin Ying Gen, Jin Gang Ci, Yang Kai Kou, Hai Jin Sha Teng, Centella (Ji Xue Cao)

Indications: Cystitis or urethritis with urgency, frequency, painful urination, hematuria, or heat-related urinary discomfort

Dosage: Chop both herbs, decoct in water, take one dose daily in three portions

Composition: Lysimachia, Corn Silk, Pyrrosia Leaf, Chicken Bone Grass, Poria, Plantago, Hai Jin Sha, Imperata Root

Functions: Clears heat, detoxifies, promotes urination

Indications: Urethritis, cystitis, acute and chronic pyelonephritis

Herbal Explanation: Hedyotis diffusa serves as the main heat-clearing herb; Dianthus, Bi Xie, Hai Jin Sha Teng, and Poria promote urination; Isatis leaf clears heat and detoxifies; Lophatherum, Huang Bai, Rehmannia clear heat and dampness; Licorice harmonizes the formula.

Dosage: Dissolve 30g in hot water, three times daily; reduce for children or follow physician instructions

Form: Granules, 15g per bag
